SEMINAR INVITATION | CARE COORDINATION FOR SENIORS:  A MULTIDISCIPLINARY TEAM APPROACH

Healthy Connections Prime, along with Palmetto Health Hospital systems will host a seminar on “Care Coordination for Seniors:  A Multidisciplinary Team Approach” on Wednesday, December 4, 2013, 9am-11am, at the Palmetto Health Richland Campus auditorium.  This educational session will have a targeted outreach to providers, specifically those within the physician community.  The featured speaker is a nationally-recognized PACE speaker and Palmetto Senior Health Consulting’s John Tucker, MHA.  This free seminar will include a panel session featuring some of our industries’ leading local providers on the multidisciplinary care approach.  In addition, continuing education Social Work credits will be available.  To attend this session, please complete the online RSVP form. | Link

 

HEALTHY CONNECTIONS PRIME PROVIDER FORUMS | PARTICIPANT SURVEY

Throughout November, we hosted regional provider forums concerning Healthy Connections Prime.  During the upcoming forums, both SCDHHS and participating CICOs addressed the impact of Healthy Connections Prime on our health care system and on specific provider types.  These targeted sessions gave over 200 providers the opportunity to ask questions regarding the implementation of this new program.  The statewide forums were held in Charleston, Columbia, and Greenville.  Following the forum, we would like to invite participants to complete an online survey to gather feedback about the sessions. | Link

HEALTHY CONNECTIONS PRIME WEBSITE EFFECTIVENESS SURVEY

In October 2013, Healthy Connections Prime launched a new consumer-friendly website.  This new website was created to provide easier navigation and usability for a variety of audiences.  Additionally, specific areas were created to house information for consumers, caregivers, advocates, providers, and CICOs.  The provider and CICO sections were launched first with the opening of the site, and other targeted sections will follow starting next month.  We would like to survey the effectiveness of the new website as we continue to add new content.  This brief survey will provide us with essential feedback on how we can revise and enhance our new site as it continues to be accessed by new visitors.  We appreciate the time taken to fill out the new survey, and all feedback provided. | Link

 

OVERVIEW OF JOINT READINESS REVIEW PROCESS FOR CICOS

Selected CICOs will undergo a joint readiness review process beginning November 2013 through June 2014.  The readiness review is one aspect of the multi-pronged oversight strategy for preserving and strengthening Medicare-Medicaid enrollees access to and quality of care under the Financial Alignment Demonstration.  CICOs were given an opportunity to submit questions and comments regarding South Carolina’s draft readiness tool.  Key components of this process include a desk review, site review, systems test and provider network validation review.  CMS has contracted with NORC at the University of Chicago (NORC) to assist in developing and conducting the readiness review process.  Both CMS and the State will make the final determination of readiness.
