SCDuE Weekly Update | August 18, 2013

CAROLINAS CENTER FOR END OF LIFE CARE | AUGUST 16, 2013

Members of the SCDuE team will provide an overview of the Demonstration for the Carolinas Center for Hospice and End of Life Care directors’ meeting on Friday, August 16, 2013.  The purpose of the session is to provide an overview of the Demonstration.  The session will explore how the hospice benefit will operate in the Demonstration as well as describe the payment structure for hospice providers under the Demonstration.  Finally, next steps will be identified for care coordination and training discussions.

 

COMMUNICATIONS WORKGROUP SESSION | AUGUST 8, 2013

The second session of the Communications Workgroup was held on Thursday, July 8, 2013.  The main purpose of the session was to discuss and confirm a timeline for the workgroup.  Based on the Communications Plan, each previously identified audience has a timeframe for education and outreach along with targeted materials.  The new branding for SCDHHS was discussed as well as its impact on the Demonstration’s recognition across the varied audiences.  A thorough list of stakeholders was reviewed to ensure accuracy and completeness.  SCDuE team offered to provide training to workgroup member organizations.  The next workgroup meeting will be held on Thursday, September 12, 2013.

 

SCDUE PRESENTATION AT SCHCA SUMMER RETREAT | AUGUST 5, 2013

Members of the SCDuE team presented at the South Carolina Health Care Association Summer Retreat to provide nursing facilities with an overview of the Demonstration.  This session explored the impact of the Demonstration on the nursing facility industry and addressed specific areas of interest including provider reimbursement rates, contracting, and care coordination.  The information was positively received by Association members and will be followed by workshops focused on contracting and other care coordination activities.  The SCDuE team, along with Nicole Mitchell Treat, will continue to provide targeted outreach to all the nursing facility associations. | Link

 

INTEGRATED CARE WORKGROUP | JULY 30, 2013

The purpose of this meeting was to provide stakeholders with an update on key operational changes related to the Demonstration. These changes included the plan selection process, the State’s enrollment strategy, and the transition of home and community based services. This session allowed stakeholders to provide feedback on these operational changes. Please note the following important dates for the CICO selection process:

August 5, 2013:  Submission date of a non-binding letter of intent to participate in Demonstration by prospective CICOs

August 22, 2013:  Submission of formal application letter and certificate of licensure from SCDOI by prospective CICOs

August 30, 2013:  Initial meetings between SCDHHS and prospective CICOs to review the MOU, project plan and readiness review requirements
