If you are interested in enrolling or want to learn more information, you can call the SC Thrive Customer Service Center at 1-800-726-8774 (TTY/TDD:711).  They are available Monday to Friday, 8:30 a.m. - 5:00 p.m.  This call is free.

To Change Your Plan Enrollment Once You Are a Member

It is your choice to stay enrolled in a Healthy Connections Prime Medicare-Medicaid Plan, switch to another plan, or leave Healthy Connections Prime altogether.  To make a change, call South Carolina Healthy Connections Choices at 1-877-552-4642, Monday through Friday, from 8 a.m. to 6 p.m.  TTY users should call 1-877-552-4670. This call is free.

For Questions About Medicare Coverage and Services

Healthy Connections Prime is a demonstration project jointly administered by the Centers for Medicare and Medicaid Services (CMS) and the South Carolina Department of Health and Human Services (SCDHHS).

Medicare is the federal health insurance program for people 65 years of age or older, some people under age 65 with disabilities, and people with end-stage renal disease (permanent kidney failure requiring dialysis or a kidney transplant).

The federal agency in charge of Medicare is the Centers for Medicare & Medicaid Services (CMS).  To contact CMS, call 1-800-633-4227 (TTY: 1-877-486-2048).  This call is free.

If you need to contact your plan, please use the contact information on the back of your Member ID card.  If you have misplaced your card, please use the number shown below for your assigned plan.  The hours of operations are 8:00 a.m. until 8:00 p.m., seven days a week.  Alternative technologies will be used on weekends and holidays.  The calls are free.
