South Carolina HCBS Statewide Transition Plan

 The South Carolina Department of Health and Human Services (SCDHHS) submitted the revised South Carolina (SC) Home and Community-Based Services (HCBS) Statewide Transition Plan to the Centers for Medicare and Medicaid Services (CMS) on Oct. 11, 2019. The revisions were necessary to receive “final approval” of the Statewide Transition Plan as required by CMS. A webinar to discuss the revised SC HCBS Statewide Transition Plan was held Aug. 20, 2019, at 11:30 a.m.

Three public meetings were held to review the revised SC HCBS Statewide Transition Plan.

  1. Aug. 13, 2019 - Simpsonville, SC
  2. Aug. 15, 2019 - North Charleston, SC
  3. Aug. 22, 2019 - Columbia, SC
