South Carolina HCBS Statewide Transition Plan

 The South Carolina Department of Health and Human Services (SCDHHS) released the revised South Carolina (SC) Home and Community-Based Services (HCBS) Statewide Transition Plan for a public comment period beginning Aug. 12, 2019. The revisions are necessary to receive “final approval” of the Statewide Transition Plan as required by the Centers for Medicare and Medicaid Services (CMS). The public can view the full public notice and revised SC HCBS Statewide Transition Plan online beginning Aug. 12, 2019, on this page. The public comment period is Aug. 12-Sept. 13, 2019.

A webinar to discuss the revised SC HCBS Statewide Transition Plan was held Aug. 20, 2019, at 11:30 a.m.

Three public meetings will also be held to review the revised SC HCBS Statewide Transition Plan.

  2. Aug. 15, 2019 - North Charleston, SC
  3. Aug. 22, 2019 - Columbia, SC

SCDHHS wants to hear from the public about the revised SC HCBS Statewide Transition Plan. Public comments must be received by 5 p.m. Sept. 13, 2019:
