Medicaid members may be interested in finding out which Medicaid-participating primary care practices and doctors in South Carolina have been designated as Patient-Centered Medical Homes (PMCH) by a national non-profit, the National Committee for Quality Assurance (NCQA). To search for a list of South Carolina practices recognized by NCQA, please access NCQA’s searchable list and select South Carolina under the list of filters by state.  Please note, that not all practices on the list participate in a Medicaid list; to verify which practices are in Medicaid, please access your health plan’s network directory or search the South Carolina Medicaid provider directory.
