Submit Reports
Hospitals may submit their Healthy Outcomes Plan reports by uploading the file using the SCDHHS Drop Link server.
HOP monthly reports are due the first Friday of the following month. Upcoming reports are due Sept. 1, Oct. 6, Nov. 3, Dec. 8, Jan. 12, Fe. 2, March 2 April 6, Maty 4 and June 8.
The Drop Link server may only be accessible by specified users. For questions, please email innovations@scdhhs.gov.
